I'm struggeling to add some new fields like phone number, because I'm not sure if you already have some class for that like "tel-box has-animation". Or do you have an example with more fields than the three of the demo (name, email, message)?
In the actual situation I got the first three field locked as well as a graphic overlay of placeholders (see attachment) - for sure I did something wron, I'm just trying to get it work :)
You're right in saying the the new css class does not have styles assigned. Perhaps you should use name-box as css class instead of tel-box since it will use its formatting? if you don't special, different styling for the telephone.
Otherwise, follow the example of name-box, email-box
